Pozycja Advances in active and passive immunotherapy for Alzheimer’s disease – a short review(Wydawnictwo Uniwersytetu Rzeszowskiego, 2016) Tabarkiewicz, JacekChoroby neurodegeneracyjne m.in. choroba Alzheimera (AD) dotykają rocznie miliony nowych pacjentów na całym świecie. Fakt ten spowodował intensywny rozwój terapii modyfikujących przebieg choroby i mogących mieć zastosowanie w AD. Dane przedkliniczne wykazały zaangażowanie układu immunologicznego w patogenezie choroby Alzheimera. Komórki mikrogleju, komórki prezentujące antygen np. komórki dendrytyczne oraz produkty aktywacji dopełniacza odgrywają aktywną rolę w neurodegeneracji. Z drugiej strony niektóre elementy układu odpornościowego mogą być wykorzystane do eliminacji złogów amyloidowych i innych nieprawidłowości związanych z AD. W związku z tym, metody pasywnej i aktywnej immunoterapii są jednym z najszybciej rozwijających się sposobów terapii choroby Alzheimera. Podawanie przeciwciał lub indukcja odpowiedzi immunologicznej przeciwko β-amyloidowi, α-synukleinie lub białku tau są intensywnie badane. Jest to związane z odkryciem, że białka te mogą być celem dla przeciwciał w przypadku ich ekspresji na błonie komórkowej lub w przestrzeni pozakomórkowej. Także stosowanie innych metod immunoterapii pasywnej (podawanie dożylnych immunoglobulin) i aktywnej (szczepionki na bazie DNA) jest związane z pozytywnymi efektami klinicznymi. Introduction: The advances in the diagnostics of eye diseases during the last decade of the 20th century and in 21st century is incomparable with the corresponding, earlier periods of development of that discipline. This can be attributed, inter alia, to the pace of the development of imaging techniquesat present at the level close to the histological picture. Objective: The latest methods of diagnosis of changes of the front and back of the eye have been presented in the paper. Material and method: The overview of the current national and foreign literature has been done, mainly for over 10 last years. The following databases have been searched: PubMed and Polska Bibliografia Lekarska.Pozycja Alleged cases of syphilis immortalized in the Krakow Altarpiece by Veit Stoss in the light of new research on the origins of the disease in Europe(Wydawnictwo Uniwersytetu Rzeszowskiego, 2016) Grzegorczyk, Wiesław; Grzegorczyk, Joanna; Grzegorczyk, KrzysztofSyphilis is believed to occur in Europe as early as at the end of the fifteenth century when it was carried to Europe from America by Columbus’ sailors in 1493 (“Columbian” theory). In the nineteenth century this theory was questioned; reference was made to historical sources, pointing to the presence of syphilis in the Old World before Columbus (“pre-Columbian” theory). In 1933 a book was published by a Polish venereologist, professor Franciszek Walter on “dermatological details” of St. Mary’s Altar in Krakow by Veit Stoss. The author drew attention to the characters carved in the altar, which can show symptoms of late congenital syphilis (including saddle nose). Stoss’s work was completed in 1489, that is four years before the first Columbus expedition returned to Europe. If Walter’s hypothesis was true, syphilis would have existed in Europe before the discovery of America. In Krakow even older historical sights, mostly sculptures, can be found Prawwhich may suggest syphilis in the presented figures. Possible symptoms of syphilis seen in medieval works of art should be differentiated from other diseases, especially leprosy. Modern studies based on modern experimental methods (paleopathological, archaeological, phylogenetic) have not given yet definitive settlement of the dispute between supporters of two main theories of the origin of syphilis in Europe.Pozycja Application of the log-linear analysis to choose determinants of disability among the elderly residents of south-eastern Poland(Wydawnictwo Uniwersytetu Rzeszowskiego, 2016) Ćwirlej-Sozańska, Agnieszka; Sozański, Bernard; Wilmowska-Pietruszyńska, AnnaIntroduction: Many variables in research in the area of medical and health sciences are qualitative in nature. A common statistical tool used to analyze them is the χ2 test. However, it does not allow us to assess the relationship between a number of variables and distinguish the factors determining the investigated phenomenon. A more accurate tool is the log-linear analysis, which enable the researcher to evaluate the dependences and interactions between the studied variables. Purpose: Description of the use of log-linear analysis on the example of the cross-sectional study on disability of the elderly. Material and methods: The assessment of disability and the choice of the factors that determine it was carried out on the results of a survey of 800 randomly selected people aged 71-80 years from the area of south-eastern Poland. The research tool was a WHODAS 2.0 questionnaire and a respondent’s particulars. The log-linear model was used for the analysis. In order to evaluate the fitting of the model, the Pearson’s χ2 and the χ2 maximum likelihood statistics, R2 and A coefficients were used . Results: Education, adjustments of a house / flat, physical exercises have a significant impact on the prevalence of disability in the study group . Conclusions: The log-linear analysis allows us to determine the effect not only of individual variables on the formation of an independent variable, but also their interactions and the determination the odds occurrence of a dependent variable according to different qualitative categories of dependent variables. The information obtained in this way is a valuable clue to take practical action to decrease or increase the severity of the studied phenomenon.Pozycja Aqua gymnastics and functional efficiency of women after 60 – pilot study(Wydawnictwo Uniwersytetu Rzeszowskiego, 2016) Walicka-Cupryś, Katarzyna; Ćwirlej-Sozańska, Agnieszka; Brzozowska-Magoń, Agnieszka; Łyczko, AnnaIntroduction and Aim: Aging of the society is a serious problem of the modern world. Elderly people often avoid physical activities which lead to sedentary lifestyle. One suitable forms of physical activity for elderly is aqua gymnastics. The aim of the study was to determine if a larger study, using the chosen methods is feasible. This study tested the hypothesis that performing a 16-week course of aqua gymnastic training would result in improved functional fitness in women aged over 60. Material and Methods: The trial included a sample of 20 women randomly selected from the 100 who responded to the application of research. The measurements such as height, weight, waistline and hip width were conducted before and after a series of exercises in water. Results: After four-month-period of exercises in water an improvement in index value was due to weight loss, a slight decrease in waistline, the basis of the Fullerton Functional Fitness and an improvement in functional efficiency of all tested women was observed. Conclusion: After 16 weeks of aqua aerobic in the group of women aged over 60 a significant functional fitness improvement was observed.Pozycja Assessing the impact of the stability training on fitness rated with the Functional Movement Screen Test in a group of professional football players(Wydawnictwo Uniwersytetu Rzeszowskiego, 2016) Kołodziej, Gabriela; Kołodziej, KrzysztofIntroduction: Football is a dynamic, ever-growing team sport. It’s fast action and spectacular character give this sport the title of the most popular in the world. However, due to its specificity it is related with a high risk of injury. An important element of injury prevention is comprehensive functional assessment of the player. The Functional Movement Screen Test is used as a tool to assess the risk of injury and basic movement patterns to determine the occurrence of disorders within the musculoskeletal system. Objective: The aim of the study is to compare the level of functional performance of football players who have stability exercises in their daily training sessions and of the players without such training. Material and Methods: The study was conducted in Rzeszow and the surrounding area on a group of 120 players. The functional assessment was performed using FMS test. Results: In the study group of players high level of functionality was observed. However, in the control group a series of tests showed an asymmetry and irregularities in the basic movement patterns. Conclusions: The study gave answers to the research questions.Pozycja Assessment of a sitting position by means of a kyphotisation indicator in the professionally active people(Wydawnictwo Uniwersytetu Rzeszowskiego, 2016) Gruca, Maria; Saulicz, EdwardIntroduction. The development of modern technology has changed human lifestyle from active to passive (sitting). According to many authors, prolonged sitting is a risk factor for overextension and painful conditions of the spine. However, it seems that this relationship can only occur in conjunction with other factors. The aim of this study is to assess a sitting position using a kyphotisation indicator constructed by the author. Material and methods. The study included 372 people living and working professionally in the Silesian Voivodeship. The research program consisted of an interview survey and measurements the length of the spine projections, which provided the calculation of a kyphotisation indicator. Results. The conducted assessment showed that the examined people receive automatically a kyphotic sitting position and this postural habit is moved to a standing position. In addition, the study highlighted the 2 types of habitual sitting positions, one with a higher kyphotisation indicator and the other with a lower kyphotisation indicator, reflecting the “dynamic” sitting and searching a comfortable (antalgic) sitting position by the researched people. Conclusions. A kyphotic sitting position is a risk factor for overextension and painful conditions of the spine, and the kyphotisation indicator can be a useful tool for further clinimetric research regarding a sitting position.Pozycja Assessment of functional capacity and the risk of falls in the elderly with mild and moderate stage dementia(Wydawnictwo Uniwersytetu Rzeszowskiego, 2016) Skubal, Anna; Sudoł, Izabela; Ciąpała, Gabriela; Ćwirlej-Sozańska, Agnieszka; Wiśniowska-Szurlej, Agnieszka; Wilmowska-Pietruszyńska, AnnaIntroduction: Dementia is a common problem among the elderly. Cognitive impairments may cause difficulty in performing basic activities of daily living. The deterioration of physical and / or mental capacity can lead to the necessity of institutional care over patients. Aim: To assess functional capacity and the risk of falls in the elderly with mild and moderate dementia. Material and Methods: The study group included 146 people aged 60-90 residing in the Residential Home as well as Care and Treatment Centre in the south-eastern Poland. Cognitive and functional capacity, balance, and the risk of falls were assessed using the following scales and clinical tests: the Mini-Mental State Examination (MMSE), Geriatric Depression Scale (GDS), Activities of Daily Living (ADL), Time Up & Go test, Tinetti test, Berg Balance Scale. Results: It has been demonstrated that individuals with dementia were characterized by reduced capacity in performing basic activities of daily living. More than half of the patients with mild and moderate dementia presented 5-times higher risk of falls than those without dementia. Moderate risk of falls was observed in 76.1% of patients with moderate dementia. Conclusions: Cognitive impairments decrease the capacity to perform basic activities of daily living. The higher the stage of dementia, the worse the balance and the greater the risk of falling. It is necessary to develop simple rehabilitation programs allowing to maintain the highest possible level of functional capacity in the elderly patients with dementia under institutional care.Pozycja Assessment of mobility, body balance and risk of fractures in the elderly(Wydawnictwo Uniwersytetu Rzeszowskiego, 2016) Ćwirlej-Sozańska, Agnieszka; Wilmowska-Pietruszyńska, Anna; Wiśniowska, Agnieszka; Guzik, Agnieszka; Drużbicki, Mariusz; Sozański, BernardIntroduction. In recent years, we have seen significant changes in the highly dynamic process of ageing amongst the population of Europe, leading to increasing life expectancy. Prolonging the life of the elderly, in which full independence and functional performance are maintained, is a challenge facing modern medicine. It is necessary to take actions to improve health and quality of life during old age. Purpose. The aim of this research is to assess the differences in mobility level and static/dynamic balance, as well as the risk of bone fractures between people aged 50 and 65 years and the elderly aged between 66 and 80 years. An analysis of the suitability of selected clinical tests is conducted to assess the above mentioned parameters. Material and methods. The study included a group of 200 randomly selected people living in Rzeszow, a city in southeastern Poland, who were aged between 50 and 80 years. All the results of the research, which ultimately involved 138 people – 103 women and 35 men – have been analysed. To assess the mobility and static/dynamic balance, the following clinical tests were used: Timed Up and Go, Functional Reach, Tandem Stance, Tandem Walk and Tandem 180° Pivot. For the assessment of fracture risk, the Fracture Risk Assessment Tool (FRAX) calculator was used, while the data were collected on the basis of direct interviews. Results. It was found that, with age, the frequency of dynamic balance, mobility disorders and the risk of bone fractures significantly increase. In a group of people aged 66 to 80 years, a decrease in the efficiency level of mobility and balance was observed in 30% of respondents on average, depending on the kind of functional test used (20-60%), in relation to people whose age did not exceed 65 years. Conclusions. It is necessary to take preventive actions, especially amongst a group of people aged over 65 years, in order to improve functional efficiency and balance, thereby reducing the risk of fractures.Pozycja Assessment of paramedics’ knowledge and skills on dealing with dental injuries(Wydawnictwo Uniwersytetu Rzeszowskiego, 2016) Lewandowski, Bogumił; Brodowski, Robert; Stopyra, Wojciech; Muster, Marek; Włodyka, Andrzej; Migut, Małgorzata; Leja, Michał; Wozniak, Krystyna; Kucaba, GrzegorzIntroduction: Traumatic teeth and mouth injuries constitute a fixed percentage of injuries. They occur as only dental injuries or accompany fractures of craniofacial skeleton or multiorgan injuries in the body. The aim of the study was to assess the paramedics’ knowledge of first aid principles in cases of traumatic dental and masticatory system injuries. Material and Methods: A survey was conducted among 138 randomly selected paramedics employed in the Medical Rescue Teams and emergency departments in Rzeszów region, who were informed about the anonymous nature of the project. A voluntarily completed questionnaire was simultaneously assumed an informed consent to participate in the study. The questionnaire consisted of two parts. The first part covered respondents’ demographic and social data, while the second part focused on the first aid conduct in tooth injuries and included 18 questions. Results: The results demonstrated that the paramedics’ knowledge of the conduct in cases of the most common dental injuries, ie. tooth knocking and dislocation, fracture of tooth crown or tooth root is unsatisfactory. Most of the respondents gave answers intuitively. Studies and statistical analysis did not confirm a significant correlation between subjects’ professional experience (p = 0.5891) and education (p = 0.1093) with their skills and knowledge of the proceedings after dental injuries. The fact that about 20% of the paramedics did not consider it necessary to broaden their knowledge in this subject seems distressing. Conclusions: In the light of conducted research, systematic training in dental first aid in terms of proceeding in teeth injuries for paramedics seems justified as well as its inclusion in the curriculum of undergraduate programme for paramedics.Pozycja Assessment of spatio-temporal gait parameters in the group of 9 years old healthy children(Wydawnictwo Uniwersytetu Rzeszowskiego, 2016) Szymczyk, Daniel; Majewska, Joanna; Szczepanik, Magdalena; Bazarnik-Mucha, Katarzyna; Snela, Sławomir; Skrzypiec, JulianIntroduction: Gait analysis can be defined as a measurement, description and assessment of human locomotion. The development of quantitative gait analysis methods and their increasing application to pathological gait assessment in children has created an urgent need for establishing normative data. The spatio-temporal gait parameters must be compared with valid reference data for the appropriate interpretation. Objective: The aim of this study was the assessment of the spatio-temporal gait parameters of 9 years old healthy children, considering the subject’s gender and the potential difference between right and left lower limb. Additional purpose of our study was to develop own reference data for the gait analysis in this age group. Material and methods: 42 healthy, nine years old children (19 girls, 23 boys), participated in this study. 3D gait analysis was performed using the computerized optoelectronic system (BTS Smart, Italy). Basic spatio-temporal gait parameters (percentage share of the stance and swing phase, right and left lower limb step lenght, as well as step frequency and average gait speed, were anylysed. Results: Our research showed no statistically significant influence of gender, and no differences between the right and left lower limbs on most of the gait parameters in the study group.Pozycja Asymptomatyczny guz neuroendokrynny okolicy krętniczo-kątniczej u 59-letniego chorego – opis przypadku(Wydawnictwo Uniwersytetu Rzeszowskiego, 2016) Wojnicka-Stolarz, Małgorzata; Gąszcz, Anna; Książek, Mariusz; Gutkowska, Dorota; Gutkowski, KrzysztofJedną z częstszych lokalizacji guzów neuroendokrynnych (Neuroendocrine Tumors, NET) przewodu pokarmowego jest jelito cienkie. W większości przypadków są to nowotwory wysoko zróżnicowane o powolnym tempie wzrastania i stanowią wyzwanie diagnostyczne dla gastroenterologów, radiologów i patomorfologów. Z klinicznego punktu widzenia NET jelita cienkiego mogą być nieczynne hormonalnie lub produkować substancje odpowiedzialne za powstawanie charakterystycznych objawów. Czułym, lecz niespecyficznym markerem tych zmian jest stężenie chromograniny A. Podstawę diagnostyki stanowią techniki obrazowe, pozwalające na lokalizację guza oraz endoskopowe, umożliwiające pobranie materiału do badania histopatologicznego i immunohistochemicznego. Chirurgiczne usunięcie zmiany jest leczeniem z wyboru, natomiast w farmakoterapii NET znajdują zastosowanie analogii somatostatyny, terapia radioizotopowa i chemioterapia. Przedstawiamy przypadek asymptomatycznego guza neuroendokrynnego zlokalizowanego w okolicy krętniczo-kątniczej u 59-letniego chorego.Pozycja Autoimmune pancreatitis mimicking malignant pancreatic tumor in 79-year-old patient – a case report(Wydawnictwo Uniwersytetu Rzeszowskiego, 2016) Partyka, Mariusz; Jednakiewicz, Mariusz; Staroń, Robert; Gutkowska, Dorota; Gutkowski, KrzysztofAutoimmune pancreatitis, (AIP) is a rare inflammatory disease of this organ distinguished by a specific clinical course and a good response to glucocorticosteroids. The disease may be limited to the pancreas itself or be part of a multi-organ autoimmune inflammatory process. The basis of diagnosis are radiological image techniques, enabling the location of organ changes, and endoscopic ultrasound examination, giving the possibility of material collection for cytological examination, supplemented by determination of the concentration of G4 subclass immunoglobulins. We report a case of autoimmune pancreatitis mimicking malignant tumor of this organ in a 79- year-old male patient.Pozycja Brunatna tkanka tłuszczowa imitująca zmianę rozrostową krezki jelita cienkiego – opis przypadku i przegląd piśmiennictwa(Wydawnictwo Uniwersytetu Rzeszowskiego, 2015) Rzucidło, Mateusz; Błotnicki, Michał; Gutkowska, Dorota; Gutkowski, KrzysztofBrunatna tkanka tłuszczowa (ang. brown adipose tissue, BAT) odgrywa kluczową rolę w procesie termogenezy bezdrżeniowej. W przeszłości uważano, iż BAT występuje tylko u noworodków i dzieci, jednak najnowsze techniki obrazowania dostarczają dowodów na obecność BAT również u osób dorosłych. Brunatna tkanka tłuszczowa lokalizuje się głównie w okolicy nadobojczykowej oraz szyi. Znacznie rzadziej obecność BAT można stwierdzić w okolicy aorty, nerek i nadnerczy oraz w obrębie sieci większej i okrężnicy. Diagnostyka obecności tego typu tkanki tłuszczowej opiera się głównie na badaniach obrazowych i badaniu histopatologicznym. W pracy przedstawiono rzadki przypadek 60-letniej chorej z dolegliwościami bólowymi w obrębie jamy brzusznej, diagnozowanej m.in. w kierunku zmiany rozrostowej krezki jelita cienkiego, która okazała się ogniskiem brunatnej tkanki tłuszczowej. Opis uzupełniono syntetycznym przeglądem piśmiennictwa.Pozycja Changes in motor development in infants participating in rehabilitation based on Vojta method(Wydawnictwo Uniwersytetu Rzeszowskiego, 2016) Czenczek-Lewandowska, Ewelina; Przygoda, Łukasz; Szklarska-Witek, Izabela; Bejster, Anna; Sadowska, LudwikaIntroduction: Vojta therapy is designed to foster beneficial change in neurophysiology of children at risk of developmental disorders. Because of its specific diagnostic and therapeutic aspects it is possible to use the method in early therapy supporting development. Purpose: Assessment of changes in motor development of infants, classified in the risk group and receiving treatment based on Vojta method. Identifying relationship between therapy effects and age at the start as well as duration of therapy. Material and method: The study was performed in a group of 62 infants, including 46 who at the time of the study were less than 6 months old (group I), and 16 who were less than 12 months old (group II). The inclusion criteria were: age below 1 year, existing motor development deficits, risk factors in pregnancy and during childbirth, and participation in rehabilitation based on Vojta therapy, as a leading method. The study was carried out with the use of child health cards and assessment tool developed by G. Banaszek. Results: After 6 months of therapy, 73% of the infants with motor development deficits improved significantly, while out of 60% of the children with abnormal postural reactions, only 16% showed no improvement. Better progress was achieved by the children who started rehabilitation before 6 months of age. Conclusions: Effectiveness of children’s rehabilitation depends on therapy duration and the age at which the treatment starts.Pozycja Duży mięśniak gładkokomórkowy żołądka imitujący guz stromalny u 53-letniej pacjentki – opis przypadku i przegląd piśmiennictwa(Wydawnictwo Uniwersytetu Rzeszowskiego, 2015) Stanek, Magdalena; Życiński, Przemysław; Książek, Mariusz; Gutkowski, KrzysztofNowotwory mezenchymalne żołądka najczęściej wykrywane są przypadkowo, w trakcie diagnostyki endoskopowej górnego odcinka przewodu pokarmowego. Większość z nich przybiera formę małych, łagodnych zmian ogniskowych nieistotnych klinicznie. W przypadku osiągnięcia dużych rozmiarów, wymagają szczegółowej diagnostyki endoskopowej, obrazowej i histopatologicznej. Pomimo dominacji guzów stromalnych wśród nowotworów mezenchymalnych żołądka, w diagnostyce różnicowej należy brać pod uwagę również inne zmiany nowotworowe, między innymi mięśniaki gładkokomórkowe. Właściwe różnicowanie jest istotne, ze względu na odmienny schemat postępowania terapeutycznego i nadzoru. W pracy przedstawiono przypadek dużego mięśniaka gładkokomórkowego żołądka, imitującego guz stromalny, u 53-letniej pacjentki. Wskazano na kluczowe elementy algorytmu diagnostycznego, ze szczególnym uwzględnieniem lokalizacji zmiany, obrazu endoskopowego oraz radiologicznego, a także badania histopatologicznego i immunohistochemicznego. In 1976, Leith and Bradley proved that strength and endurance of the respiratory muscles in young healthy volunteers could be successfully trained using respiratory muscle endurance training. In 1992 Urs Boutellier and Christina Spengler’s cooperation resulted in the construction and distribution of SpiroTiger training device. Aim of the study. To analyze available data on monitoring and documenting changes after the respiratory muscles endurance training. Presentation of changes after specific training in the athletes, healthy and chronically ill individuals. Material and methods. Literature Research and the PubMed database analysis using the following keywords: respiratory muscles endurance training, normocapnic hyperventilation, SpiroTiger. Conclusions. Monitoring and documentation of changes in the lung and respiratory muscle function and endurance under the influence of the training can be conducted in a manner consistent with evidence-based medicine. It has been demonstrated that hypertension diagnosed in childhood persists in adulthood. Therefore, knowledge of the risk factors, prophylaxis and treatment of hypertension are crucial due to its serious medical and social consequences. Early identification of risk factors for hypertension allows to implement targeted preventive actions leading to the modification of habits connected with lifestyle, which in turn may lead to a reduction in the incidence of the disease or reduce its effects. The aim of the paper is to characterize the relationship between the occurrence of excessive body mass and hypertension in adults and in children and adolescents. Material and methods. A review of Polish and foreign literature mainly from the last 10 years. The following databases were searched: PubMed, EBSCO, Science Direct, Termedia, Polish Medical Bibliography.
