Abstract
Wstęp: Rehabilitacja poudarowa ma na celu przywrócenie samodzielności pacjentów, jednak dowody dotyczące skuteczności treningu propriocepcji stawu kolanowego wspomaganego robotycznie pozostają ograniczone.
Cel: Ocena wpływu treningu propriocepcji na chód, równowagę, czucie głębokie i stan funkcjonalny pacjentów we wczesnym okresie po udarze mózgu.
Materiał i metody: Badaniem objęto 56 pacjentów do 3 miesięcy po udarze niedokrwiennym mózgu, podzielonych na grupę kontrolną (n=28), realizującą standardową rehabilitację, oraz badaną (n=28),
dodatkowo objętą treningiem propriocepcji z wykorzystaniem urządzenia LUNA EMG. Ocenę przeprowadzono przed i po 3 tygodniach terapii.
Wyniki: W grupie badanej uzyskano istotną poprawę prędkości chodu (p<0,001; dz=-0,90), długości cyklu chodu i kadencji. W obu grupach odnotowano istotną poprawę w skali Barthel i FAC.
Wnioski: Dodatkowy trening propriocepcji wspomagany robotycznie może poprawiać chód i równowagę dynamiczną po udarze mózgu. Konieczne są dalsze badania nad optymalizacją protokołu terapii.
Introduction: Stroke rehabilitation aims to restore independence, but evidence on robot-assisted knee proprioceptive training remains limited. Aim: To assess its effects on gait, balance, proprioception and functional status early after stroke. Methods: Fifty-six patients within 3 months of ischaemic stroke were assigned to standard rehabilitation (n=28) or standard rehabilitation plus proprioceptive training with LUNA EMG (n=28). Gait, balance, knee proprioception and functional status were assessed before and after 3 weeks. Results: The experimental group showed a significant improvement in gait speed (p<0.001; dz=-0.90), with benefits also observed in gait cycle length and cadence. No between-group differences were found in gait phase durations. Balance improved in both groups, with greater gains in the experimental group on the Berg Balance Scale (p=0.032) and Timed Up and Go test (p=0.002). Changes in proprioception were limited. Both groups improved significantly on the Barthel Index and Functional Ambulation Categories. Conclusion: Adding robot-assisted proprioceptive training to standard rehabilitation may improve gait and dynamic balance after stroke, although further research is needed to optimise the protocol.
