Abstrakt
Wrodzone zarośnięcie przełyku jest najczęstszą wadą
wrodzoną tego narządu. Polega ona na niewykształceniu
fragmentu przełyku, w wyniku czego nie ma zachowanej
ciągłości, a jeden lub oba jego końce kończą się ślepo,
bądź komunikują się z tchawicą. Częstość występowania
atrezji wynosi 1/2500-3000 urodzeń. Może ona
występować w postaci izolowanej lub zespołowej z co
najmniej jedną wadą towarzyszącą. Embriologia przełyku,
jak i etiopatogeneza wrodzonego zarośnięcia są jak dotąd
słabo poznane. Nie zidentyfikowano głównego czynnika
sprawczego, ale przypuszcza się, że postać izolowana
indukowana jest przyczyną wieloczynnikową (genetyczną
lub środowiskową).
Wyróżnia się 5 głównych typów anatomicznych wady.
Najczęstszym jest postać wrodzonego zarośnięcia przełyku
z przetoką przełykowo-tchawiczą dolną (typ III wg Ladda
- około 85% przypadków).
W celu oceny ryzyka oraz możliwości porównawczych
efektów leczenia wprowadzono skale rokownicze:
Waterstona, Montrealską oraz najczęściej używaną - skalę
ryzyka Spitza, w której głównymi kryteriami oceny dla
przeżycia są masa ciała dziecka oraz obecność wady serca.
Próby chirurgicznego leczenia wrodzonego zarośnięcia
przełyku sięgają drugiej połowy XIX. 14 marca 1941 roku
Cameron Haight, jako pierwszy w historii, z sukcesem
zoperował pacjenta według obecnie obowiązujących
zasad.
Postępy w chirurgii, neonatologii oraz intensywnej opiece
noworodkowej spowodowały, iż aktualnie atrezja przełyku
cechuje się stosunkowo niską śmiertelnością, ale nadal
relatywnie częstą obecnością powikłań późnych; tj.
zwężenia w miejscu zespolenia przełyku, odpływu
żołądkowo-przełykowego oraz zaburzeń połykania.
Podjęcie badań nad wczesnymi i odległymi wynikami
leczenia wrodzonego zarośnięcia przełyku daje możliwość
ich dogłębnej analizy, porównania oraz wymiany
informacji, co ma na celu poprawę leczenia oraz jakości
życia dzieci obarczonych tą wadą.
Za cele pracy przyjęto:
• • Ocenę wyników leczenia dzieci z wrodzonym
zarośnięciem przełyku leczonych w Oddziale Chirurgii
Dziecięcej Wojewódzkiego Szpitala nr 2 w Rzeszowie.
• • Analizę związku pomiędzy powikłaniami
wczesnymi, parametrami urodzeniowymi noworodków,
wadami towarzyszącymi, a zastosowanym leczeniem.
• • Analogiczną analizę w celu oceny przeżycia.
• • Określenie, które z analizowanych cech
związanych z przebiegiem leczenia mogą stanowić
czynnik ryzyka dla wystąpienia klinicznie najczęstszych
powikłań późnych takich jak:
• 1. Zaburzenia połykania.
• 2. Zwężenie przełyku.
• 3. Refluks żołądkowo przełykowy.
Na podstawie dokumentacji medycznej do badania
zakwalifikowano 60 dzieci operowanych w Oddziale
Chirurgii Dziecięcej w Rzeszowie w latach 1995-2014.
Poddano analizie historie chorób, wyniki badań
diagnostycznych, dokumentację medyczną z leczenia w
innych ośrodkach oraz protokoły operacyjne.
Badana grupa dzieci liczyła 32 chłopców i 28
dziewczynek. Średnia masa ciała wynosiła – 2355g (zakres
890-4030g), a przeciętny wiek ciążowy - 36 tygodni
(zakres 28–41 tygodni). W pierwszej minucie życia dzieci
uzyskiwały średnio 6,7 pkt w skali APGAR. W
analizowanej grupie noworodków u 56 stwierdzono III typ
zarośnięcia przełyku wg Ladda, u 2-typ I, typy IV i V
wystąpiły jednokrotnie. Wg klasyfikacji rokowniczej
Spitza do grupy I ryzyka zakwalifikowano 41 dzieci, do
grupy II - 16, III - 3. Postać złożoną wrodzonego
zarośnięcia przełyku rozpoznano u 35 dzieci (58,3%).
Najczęściej stwierdzano wrodzone wady serca, układu
pokarmowego, układu moczowo–płciowego oraz układu
kostno–stawowego. Zaburzenia chromosomalne
stwierdzono u 3 dzieci (2 z zespołem Edwardsa, 1 z
zespołem Downa). Najczęstszym zabiegiem operacyjnym
(wykonanym u 49 noworodków) było pierwotne
zespolenie przełyku z zaopatrzeniem przetoki przełykowo tchawiczej dolnej poprzez torakotomię prawostronną z
dostępu pozaopłucnowego.
Spośród 60 dzieci zmarło 12. Przyczyną śmierci w 11
przypadkach była posocznica. Zmarły wszystkie dzieci z
zaburzeniami chromosomalnymi. Z grupy ryzyka I wg
Spitza zmarło 3 dzieci (9,5%), z II – 6 (33%). Zmarły
wszystkie dzieci z grupy III (n-3, 100 % ) 98
Powikłania późne analizowano u pacjentów zaproszonych
listownie do badania kontrolnego (spośród żyjących 48
pacjentów do Poradni Chirurgii Dziecięcej zgłosiło się 33
dzieci). Polegało ono na analizie dokumentacji medycznej,
badaniu podmiotowym (ankieta) oraz badaniu fizykalnym
z wykonaniem diagnostyki ultrasonograficznej jamy
brzusznej. Średni czas przeprowadzonego badania
kontrolnego wyniósł 125 miesięcy. Zaburzenia połykania
występowały u 54,5% dzieci, zwężenie przełyku u 42,4%,
a refluks żołądkowo-przełykowy u 42,4%. W trakcie
badania kontrolnego rozpoznano u 5 pacjentów,
niezdiagnozowane uprzednio, wady dodatkowe.
Uzyskane wyniki poddano analizie statystycznej przy
użyciu testu W. Shapiro–Wilka, t-Studenta, U. Manna–
Whitneya. Do oceny przeżycia zastosowano estymator
Kaplana–Meiera, testy log–rank, F. Coxa oraz model
proporcjonalnego hazardu Coxa.
W pracy przyjęto 5% ryzyko błędu (dla p < 0,05 różnicę
uznano za istotne statystycznie).
Śmiertelność w badanej grupie wyniosła 20%. Istotnie
statystycznie niższy odsetek przeżycia zaobserwowano u
noworodków z niską masą urodzeniową, w przypadku
wystąpienia posocznicy, przy konieczności włączenia
przedoperacyjnej sztucznej wentylacji oraz w przypadkach
pooperacyjnej niewydolności oddechowej.
Noworodki o prawidłowej masie ciała były bardziej
dojrzałe, osiągały wyższą punktację w skali APGAR,
miały krótszy czas operacji i pobytu w OIOMI-e, krótszy
czas sztucznej wentylacji oraz były w mniejszym stopniu
narażone na posocznicę.
Noworodki urodzone o czasie miały wyższą masę
urodzeniową, rzadziej rodziły się drogą cięcia cesarskiego,
były szybciej karmione doustnie oraz wcześniej
wykonywano u nich badanie endoskopowe.
Dzieci, u których stosowano przedoperacyjną sztuczną
wentylację miały niższą masę urodzeniową, były
niedojrzałe oraz uzyskiwały mniejszą ilość punktów w
skali APGAR.
Zwężenie przełyku występowało istotnie częściej u
pacjentów z wadami serca i układu pokarmowego. Ze
względu na wysoki odsetek wad towarzyszących
zarośnięciu przełyku oraz duże ryzyko późnych powikłań
atrezji wszystkie dzieci obarczone tą wadą należy poddać
kompleksowemu postępowaniu diagnostycznemu oraz
otoczyć je wysokospecjalistyczną opieką lekarską.
Congenital esophageal atresia is the most frequent congenital defect of this organ. In this condition a part of the esophagus does not develop, as a result of which the continuity is broken and one or both of its’ ends are closed blind or are attached to the trachea. The frequency of atresia’s occurrence amounts to 1/2500- 3000 births. It may occur in an isolated or syndromic form with at least one congenital malformation. Embryology of the espophagus as well as the ethiopathogenesis of esophageal atresia have been poorly recognized so far. The main causative factor has not been identified, however supposedly, the isolated form is induced by the multi factor cause (genetic or environmental). Five main anatomic types of the disorder can be distinguished. The most common is the esophageal atresia with a distal tracheoesophageal fistula (type III according to Ladd classification – approximately 85% of cases). In order to evaluate the risk and the possibilities of comparative results of the treatment, prognostic scales were introduced: Waterson, Montreal and the most commonly used one – the Spitz classification, where the main evaluation criteria of survival are the baby’s birth weight and the presence of cardiac abnormalities. The attempts of surgical treatment of esophageal atresia date back to the second half of 19th century. 14th March 1941 Cameron Haight was first in history to successfully operate on a patient in compliance with the current principles. Progress in surgery, neonatology and the neonatal intensive care resulted in the relatively low mortality in esophageal atresia, however still relatively frequent presence of late complications; i.e. anastomotic stenosis, gastro-esophageal leakage and ingestion disorders. Undertaking research on the short and long-term outcomes of treatment of esophageal atresia gives an opportunity of a comprehensive analysis, comparison and exchange of information which aims to improve the treatment as well as life quality for children suffering from this disorder. 100 The aims of the thesis: • The evaluation of the treatment of children with esophageal atresia treated in the Paediatric Surgery Unit of Provincial Hospital No 2 in Rzeszów. • The analysis of a correlation between the early complications, birth parameters of newborns, congenital malformations and the applied treatment. • The parallel analysis in order to evaluate the survival. • Determining, which of the analysed characteristics connected with the course of the treatment may pose a risk factor for the occurrence of the clinically most frequent late complications such as: 1. Dysphagia. 2. Espohageal stenosis. 3. Gastro-esophageal reflux. On the basis of the medical documentation, 60 children who underwent the surgery in the Paediatric Surgery Unit of Provincial Hospital No 2 in Rzeszów between 1995 and 2004 were qualified for the study. Medical records, results of diagnostic tests, medical documentation from the treatment in other centres as well as surgical protocols were analysed. The study group comprised of 32 boys and 28 girls. The mean body mass amounted to 2355g (range 890-4030g) and the mean gestational age – 36 weeks (range 28-41 weeks). In the first minute of life, the children obtained on average 6,7 points in APGAR scale. In the analysed group of newborns, in 56 cases type III of esophageal atresia according to Ladd classification was diagnosed, in 2 – type I, types IV and V happened once. According to Spitz prognostic classification, 41 children were qualified for the I risk group, 16 to group II and 3 to group III. Syndromic esophageal atresia was diagnosed in 35 children (58,3%). The most commonly diagnosed defects were cardiac malformations, digestive, genito-urinary tract and osteo articular defects. Chromosomal disorders were diagnosed in 3 children (2 with Edward’s Syndrome, 1 with a Down Syndrome). The most frequently performed surgical procedure (in 49 newborns) was the primary esophageal anastomosis with distal tracheo-esophageal fistula by means of right-sided thoracotomy with extrapulmonary access. 12 children out of 60 died. The cause of death in 11 cases was sepsis. All the children with chromosomal disorders died. 3 children from the I risk group according to Spitz classification died (9,5%), 6 from the II (33%). All the children from III group died (n-3, 100%). 101 Late complications were analysed in patients invited to the follow-up examination by post (out of 48 patients alive, 33 children reported to the Paediatric Surgery Outpatient Clinic). The medical documentation was analysed, clinical examination (the survey) and physical examination with the ultrasound diagnostics of the abdomen were performed. The mean time of the follow-up examination amounted to 125 months. Dysphagia was observed in 54,5% of children, esophageal stenosis in 42,4% and gastro-esophageal reflux in 42,4%. In the course of the follow-up examination in 5 patients, additional malformations, previously undiagnosed, were found. The obtained results underwent statistical analysis by means of W Shapiro-Wilk test, t-Student, U Mann Whitney. For the survival evaluation, Kaplan-Meier estimate was applied, log-rank tests, F.Cox as well as Cox proportional hazard model. The estimated risk of error in the study was 5% (for p < 0,05 the difference was considered statistically significant). The mortality in the study group reached 20%. Statistically significant lower survival percentage was observed in newborns with a low birth weight, in case of sepsis, with the necessity to introduce preoperative artificial respiratory support and in cases of postoperative respiratory failure. The newborns with a normal body mass were more mature, obtained higher scores in APGAR scale, had a shorter time of the surgery and stay in ICU, artificial respiratory support and were less prone to sepsis. The newborns born on time had a higher birth weight, were less frequently delivered by caesarean section, were faster fed orally and had an endoscopic examination performed earlier. Children in which preoperative artificial respiratory support had a lower birth weight, were immature and obtained a lower score in APGAR scale. Esophageal stenosis occurred significantly more often in patients with cardiac malformations and digestive system defects. Due to the high percentage of congenital malformations accompanying the esophageal atresia and the high risk of late complications of atresia, all the children suffering from this disorder should be subject to comprehensive diagnostic management and should be treated with the utmost professional medical care.
