Xerostomia is one of the most common and disturbing adverse effects of systemic diseases and their therapies. This complication markedly increases the risk for dental caries, difficulties with chewing, swallowing and sleep disorders with a significant impact on the patient’s quality of life. Sjögren’s syndrome (SS) is a systemic autoimmune disease that primarily affects the exocrine glands, resulting in dryness of the mouth due to lymphocytic infiltration of the salivary glands. Aim. The aim of this paper is to present the current recommendations in diagnosing and treating SS-related xerostomia. Material and methods. Analysis of literature Results. For the assessment of SS-related xerostomia, only an unstimulated salivary flow with rates of 0.1 mL/min is included in the current SS classification criteria. Saxon test, sialography, ultrasonography of salivary glands play supporting function. Treatment of SS -related xerostomia includes an application of secretagogues and the implementation of specific dental prophylaxis measures. Adjuvant therapies include herbal remedies, photobiomodulation, and acupuncture. Conclusion. Treatment of SS requires multidisciplinary care. There is no fully effective treatment of xerostomia that provides immediate and long-lasting results.Pozycja Evaluation of salivary alpha amylase activity in smokers with periodontitis, Khartoum state, 2023(Publishing Office of the University of Rzeszow, 2023) Allah, Fatima Abd Alwhab Awad; Ahmed, Hafsa Ahmed ElrheimaIntroduction and aim. Smoking widely affect oral health, including its role in the development of periodontitis. Saliva contains an antioxidant system and various enzymes. The study was designed to evaluate the activity of salivary alpha amylase among individuals who smoke and have periodontitis. Material and methods. A total of 100 participants were included, with 50 cases (cigarette smokers with periodontitis) and the remaining 50 nonsmokers with healthy periodontium as the control group. Saliva samples were collected to measure salivary alpha amylase activity. Results. Smokers with periodontitis had significantly higher levels of salivary alpha amylase compared to the control group (177.96±14.5 vs 94.04±19.6 IU/mL, p<0.001). Additionally, there was a weak negative correlation between the level of alpha amylase and the age of the patients (p=0.01, r=0.376). However, no correlation was found between the level of alpha amylase and the duration of smoking (p=0.584, r=0.079). Conclusion. There is a significant increase in salivary alpha amylase levels among smokers with periodontitis. No correlation was found between age and salivary alpha amylase levels. However, a weak positive correlation was observed between the duration of smoking and salivary alpha amylase activity.Pozycja Saliva pH testing in predicting dental caries in children aged 7–10 years(Wydawnictwo Uniwersytetu Rzeszowskiego, 2015) Fudali-Walczak, Magdalena; Raba, Grzegorz; Obłoza, BeataWstęp: Próchnica zębów jest najbardziej rozpowszechnioną chorobą narządu żucia. Według raportów WHO oraz FDI nie udało się zmniejszyć epidemii próchnicy. W Polsce zachorowalność dzieci na próchnicę zębów utrzymuje się na wysokim poziomie i jest jedną z najwyższych w Europie. Cel: 1. Ocena wartości badania pH śliny jako testu screeningowego w modelowaniu ryzyka wystąpienia choroby próchnicowej zębów u dzieci. 2. Ocena korelacji wartości pH śliny ze stanem jamy ustnej wyrażonym za pomocą wskaźnika PUW. Materiał, metody: Badaną grupę stanowiło 128 dzieci w wieku 7–10 lat. Badanie polegało na: 1. Dokonaniu przeglądu uzębienia, przy pomocy podstawowych narzędzi stomatologicznych, 2. Ocenie pH śliny pacjenta za pomocą papierka lakmusowego. Na podstawie przeprowadzonych przeglądów jamy ustnej, wyznaczono dla każdego pacjenta wskaźnik PUW. Wyniki: 1. Występowanie próchnicy – 75,8%. 2. Odsetek dzieci z usuniętymi zębami – 18%. 3. Odsetek dzieci leczonych - 79,7%. 4. Wartość korelacji współczynnika PUW oraz pH śliny: 0,693; -0,730 (p = 0,000). Wnioski: 1. Stan zdrowia jamy ustnej dzieci w wieku 7–10 lat jest zły. U 76% stwierdza się próchnicę, 18% dzieci ma już usunięty przynajmniej jeden ząb. 2. Tumour necrosis factor-alpha (TNF-α) belongs to the cytokine family TNF/TNFR. As a multifunctional cytokine, TNF-α plays a significant role in diverse and a variety of cellular events such as cell survival, proliferation, differentiation, and death. As a pro-inflammatory cytokine, TNF-α acts as a bridge between inflammation and carcinogenesis. Receptor for advanced glycation end products (RAGE) are cellular receptors belonging to the immunoglobulin superfamily. As one of the primary mediators of innate immunity, acute and chronic inflammatory disorders, and certain cancers, RAGE signaling plays an important role. The aim of the present study is to analyze the prognostic significance of salivary TNF-α and RAGE in oral squamous cell carcinoma. Material and methods. A study was conducted testing saliva samples collected from ten patients with well-differentiated and moderately differentiated oral squamous cell carcinomas. To determine the levels of TNF-α and RAGE in unstimulated saliva from patients, an ELISA kit from RAY BIOTECH was used for the study, and the readings were read at 450 nm. Statistical analysis was performed using SPSS software. Version 23 of SPSS was used to plot the standard curve. Statistical comparisons were done using Mann-Whitney U test and ROC analysis. Results. Salivary TNF-α and RAGE in patients were considered to be induced by radiotherapy at a higher level in moderately differentiated squamous cell carcinoma when compared to well differentiated squamous cell carcinoma. Thus, there is an increase in the induced Salivary TNF-α and RAGE levels by radiotherapy with increase in the histological stages of oral squamous cell carcinoma. The statistical analysis also proved the same. Conclusion. Hence salivary TNF-α and RAGE may be used as a biomarker for oral cancer to predict the prognosis.
