Abstrakt
Wstęp
Wystąpienie porodu przedwczesnego wiąże się
ze znacznymi komplikacjami ze strony nieprzygotowanego do
życia pozamacicznego organizmu, a także jest główną przyczyną
śmiertelności noworodków. Znaczącym punktem uchwytu
wydaje się być system endokanabinoidowy, mający
udowodniony wpływ w patogenezie reakcji zapalnej.
Cel pracy
Zbadanie zależności pomiędzy ekspresją receptorów
kanabinoidowych w obrębie łożyska po porodzie, a problemem
porodu przedwczesnego.
Materiał i Metody
Badanie przeprowadzono na materiale wieloośrodkowym 150
kobiet rodzących w terminie od marca 2004 do grudnia 2012
roku. U 115 kobiet wystąpił poród przedwczesny i stanowiły
Grupę Badaną, u 35 kobiet poród odbył się w fizjologicznym
terminie i określono Je mianem Grupy Kontrolnej.
W celu określenia ekspresji receptorów kanabinoidowych CB1, CB1a oraz CB2 w łożyskach, bezpośrednio po zakończeniu III
okresu porodu i wydaleniu popłodu, z łożysk pobierano 4
wycinki; po jednym z każdego kwadrantu, z miejsc
makroskopowo niezmienionych. Pobraną tkankę łożyskową
przebadano używając następujących technik: izolacji RNA,
odwrotnej transkrypcji (RT) i reakcji łańcuchowej polimerazy w
czasie rzeczywistym (Real-Time PCR) w celu oceny ekspresji
receptorów kanabinoidowych w badanym materiale.
Wyniki
Przeprowadzona analiza statystyczna pomiędzy badanymi
grupami wykazała, że ekspresja receptora CB2 była niższa w
łożyskach pochodzących od kobiet, które urodziły
przedwcześnie. W grupie badanej występowanie porodu
przedwczesnego w wywiadzie, ciąż obumarłych w wywiadzie i
ciąż zakończonych cięciem cesarskim było tym częstsze, im
niższa była ekspresja receptorów CB1 i CB1a w łożysku
pacjentek z grupy badanej. Wymienione powikłania są
powszechnie znanymi czynnikami ryzyka porodu
przedwczesnego w kolejnych ciążach.
Wnioski
1. W patogenezie porodu przedwczesnego może mieć
znaczenie ekspresja receptorów kanabinoidowych.
Im niższa ekspresja receptora CB2 w łożysku, tym
częściej występuje poród przedwczesny.
2. Stwierdzona w badaniu korelacja ekspresji
receptorów CB1, CB1a, CB2 z powikłaniami przebiegu
ciąży takimi jak: występowanie u kobiet rodzących w
przeszłości porodu przedwczesnego, ciąż
obumarłych, krwawień w przebiegu ciąży,
podwyższonej ciepłoty ciała podczas porodu oraz
zakażeń układu moczowego, uzasadnia wniosek, iż
należy pogłębić diagnostykę ekspresji receptorów
kanabinoidowych u kobiet ciężarnych na różnych
etapach ciąży.
Introduction The occurrence of this pathology is associated with significant complications for not prepared to an extrauterine life newborn and is also the leading cause of infant mortality. A significant fixation point seems to be the endocannabinoid system, which has a proven effect on the pathogenesis of the inflammatory response. Objectives Investigation of the relationship between cannabinoid receptor expression within the placenta after delivery and the problem of preterm delivery. Material and methods The study was conducted on multicenter material of 150 women giving birth in the period from March 2004 to December 2012. 115 women were diagnosed with premature delivery and were qualified to the Study Group, 35 women had a delivery at the physiological time and were qualified to the Control Group. In order to determine the expression of cannabinoid CB1, CB1a, and CB2 receptors in the placenta, immediately after the end of the 3rd delivery and excretion of the postpartum, 4 sections were taken from the placenta: one from each quadrant, from macroscopically unchanged locations. The following techniques were used: RNA isolation, reverse transcription (RT) and RealTime PCR to assess the expression of cannabinoid receptors in the tested material. Results In statistical analysis following differences between the study group and control group showed that CB2 receptor expression was lower in the placenta from women who undergo preterm delivery. In the study group, the history of preterm labor, history of intrauterine fetal deaths, and pregnancies terminated by Caesarean section correlated with lower expression of the CB1 and CB1a receptors in the placenta in patients from the study group. These complications are well-known risk factors for preterm labor in subsequent pregnancies. Conclusions 1. Expression of cannabinoid receptors could be an essential tool in the pathogenesis of preterm labor. The lower CB2 receptor expression more often occurs in the placenta delivered after preterm labor. 2. The correlation between CB1, CB1a, CB2 receptors expression and pregnancy complications found in the study, such as the occurrence of preterm delivery occurrence in the past, intrauterine death of the fetus in the past, vaginal bleeding during pregnancy, increased body temperature during delivery, and urinary tract infections, justifies that the diagnosis of cannabinoid receptor expression in pregnant women at various stages of pregnancy should be performed.
