Abstrakt
Częstość występowania otyłości i nadwagi oraz chorób z
nimi związanych w krajach rozwiniętych stała się
poważnym problemem zarówno zdrowotnym jak i
społecznym. Celem pracy było określenie zależności
pomiędzy dietą, stylem życia, składem masy ciała,
spoczynkową przemianą materii oraz stężeniem
mediatorów związanych ze stanem zapalnym
(interleukin: IL-1, IL-6, IL-17A, IL-17F, IL-17C, IL-21, IL22, IL 23, IFN γ, TNF α) i otyłością (grelina,
adiponektyna, rezystyna, leptyna, glukoza, glukagon,
insulina). Materiał i metody: Badaniami objęta została
grupa pełnoletnich pacjentów z BMI ≥ 30 kg/m2
(otyłość wg. Klasyfikacji WHO). Przebadanych zostało
84 osób, w tym 62 kobiety, 22 mężczyzn, w wieku od 18
do 74 lat. Każdy z badanych miał pobraną krew w celu
dalszej analizy, przeprowadzaną ankietę żywieniową i
wywiad 72 godzinny, wykonywaną analizę składu ciała a
także badanie kalorymetrii pośredniej. Zarówno
rozmieszczenie tkanki tłuszczowej i mięśniowej jak i ich
ilość jest skorelowana z profilem mediatorów
zapalnych. Dieta zarówno pod kątem produktów jak i
poszczególnych składników miała znaczenie w nasileniu
stanu zapalnego, który towarzyszy otyłości. Zarówno
używki jak i wysiłek fizyczny zmieniały profil cytokin i
adipokin. Low grade inflammation nie wpływał na
spoczynkową przemianę materii.
The prevalence of obesity, overweight and disease associated with them in developed countries has become a serious health and social problem. The aim of the study was to determine the relationship between diet, lifestyle, body mass composition, resting metabolism and the concentration of mediators associated with inflammation (interleukins: IL-1, IL-6, IL17A, IL-17F, IL-17C, IL-21, IL-22, IL-23, IFN γ, TNF α) and obesity (ghrelin, adiponectin, resistin, leptin, glucose, glucagon, insulin). Material and methods: The study included a group of adult patients with BMI ≥ 30 kg/m2 (obesity according to WHO classification). 84 people, i.e. 62 women, 22 men, aged from 18 to 74 years were subjected to examinations. Each of the subjects had blood taken for further analysis, a nutritional questionnaire and a 72-hour interview, body composition analysis and indirect calorimetry tests. Both the distribution of adipose tissue and muscle tissue as well as their amount is correlated with the profile of inflammatory mediators. The diet, both in terms of products and individual ingredients, was important in the intensification of inflammation that accompanies obesity. Both stimulants and exercise changed the profile of cytokines and adipokines. Low grade inflammation did not affect resting metabolism.
